IV. CONSENT TO PERSONAL DATA PROCESSING
By ticking the box “I CONSENT TO THE PROCESSING OF PERSONAL DATA ”, I agree that the Czech National Bank, registered address Na Příkopě 28, 115 03 Prague 1, company ID 48136450, as a personal data controller (“controller ”) pursuant to Articles 4(n), 5(4) and 11 of Act No. 101/2000 Coll., on Personal Data Protection and the Amendment of Other Acts, as amended (“Act No. 101/2000 Coll.”), may process my personal data for the purposes of registration for the Eurobadminton 2018 tournament (“tournament”) organised by the controller and the related accommodation for registered tournament participants. The controller will also process my personal data for the purposes of processing the results of individual disciplines.
The controller will process the following personal data: family name, first name, gender, date of birth, e-mail address, passport or ID number and contact person telephone number. The personal data will be processed by the controller both manually and in an automated manner. The controller will store the personal data for a period of six months.
The personal data of registered participants (family name, first name, gender, date of birth, e-mail address, passport or ID number and contact person telephone number) will be transferred to TOP HOTELS GROUP a.s., registered address Blažimská 1781/4, 149 00 Prague 4 - Chodov, company ID 26443392, as the controller’s contractual partner providing accommodation for tournament participants.
Personal data (family name, first name, gender) will be transferred to MIRA Business Group s.r.o., registered address Květnice, Bedrníková 331, 250 84, company ID 03478343, as the controller’s contractual partner organising the tournament, i.e. scheduling players into matches and processing the results of the tournament.
I am aware that I am entitled to revoke the consent to personal data processing at any time. Consent can be revoked by sending an e-mail to: conference@cnb.cz.
If the consent to personal data processing is revoked, I am aware that it will then be impossible to process the registration for the tournament. If I requested accommodation provided by the controller and revoke consent to personal data processing, I am aware that the controller will not be able to provide accommodation.
I hereby confirm that I have been advised of my rights arising from the processing of personal data, notably the right of access to personal data, the right to have personal data rectified, the right to withdraw consent to the processing of personal data, the right to request remedy of a situation which is contrary to the law, especially in the form of suspension of handling of personal data and the rectification, amendment or removal of personal data, and the right to file a complaint with the Office for Personal Data Protection. Pursuant to the General Data Protection Regulation – Regulation (EU) 2016/679 of the European Parliament and of the Council (“GDPR”), which will be directly applicable in the territories of all Member States, a natural person also has the right of erasure of personal data and the right of limited processing of personal data under the conditions laid down in GDPR.
